Outpatient Lacrimal Procedures

Purpose
To showcase the lacrimal-related Office-Based Procedures (OBP) performed in a single tertiary institute over the last 2 years.
Methods
Retrospective analysis of lacrimal-related OBP performed from 2022 to 2023.
Results
Over 2 years, 609 oculoplastic-related OBPs were done. 40 (6.7%) lacrimal drainage procedures, excluding nasoendocopic evaluation of the lacrimal system, were done in a tertiary institute outpatient oculoplastic procedure clinic. Of the lacrimal procedures, 7 (17.5%) patients had bilateral procedures done whilst 33 (82.5%) were unilateral. 25 patients (75.8%) were females and mean age was 61.5 +/- 14.3 years. Ethnicity: Chinese 30 (62.5%), Indian 5(12.5%), Others 3(7.5%) and Malay 2(5.0%). Breakdown of procedures is as follows: 20 (50.0%) Endoluminal Duct Recanalisation (ELDR), 8 (20.0%) 3 snip punctoplasty, 9 (22.5%) punctum closure and 3 (7.5%) canaliculotomy. Of the ELDRs, all except one proceeded with bicanalicular intubation with silicone tubing (Crawford tube) post-recanalisation. Bilateral concurrent ELDRs were performed for 2 patients. These procedures were done under endoluminal anaesthesia (ELA) and were well tolerated. Of the 8 that had ELDRs done in 2022, 6 (75.0%) had good outcomes at post-procedure year one, 1 (12.5%) had distal canalicular stenosis and 1 (12.5%) patient had functional nasolacrimal duct obstruction.
Conclusion
Office-based procedures (OBP) are an important step in improving accessibility to and affordability of healthcare. Surgery-related costs has been shown to be 70% higher than OBP (operating room fee, anesthesiology fee, medication fees etc). Moreover, in our centre, monthly dedicated lacrimal clinics are conducted in collaboration with Ear, Nose, Throat specialists. This joint effort allows us to co-manage lacrimal and nasal disorders and ensures optimal care for our post-surgery patients.
