The clinical and radiological characteristics of the punctal system in patients with epiphora compared to healthy participants.

Purpose
To examine the clinical and radiological characteristics of patients with epiphora compared to healthy participants
Methods
This is a prospective, comparative clinical study. Patients with complaints about epiphora who planned for punctoplasty surgery, paitents with complaints about epiphora who planned for DCR surgery and healthy participants participated in the study. The following data was collected and compared: epidemiological data, the degree of epiphora (Munk score), punctal size as measured by anterior segment optical coherence tomography (AC- OCT).
Results
Thirty six healthy participants (18 men, 18 women), 27 patients who planned for punctoplasty surgery (6 men, 21 women) and 20 patients who planned for DCR surgery participated in the study (9 men, 11 women). Their mean Munk score was 0.2 versus 3.8, 3.9 respectively (p=0.042). The length of the punctal surface, the length of punctum at 100 μm below the surface, the wide of punctum and the area were significantly higher in the healthy participants group compared to the patients with epiphora who planned for punctoplasty surgery or DCR surgery (p<0.01, p<0.01, p<0.01, p<0.01 respectively). However, the punctum surface and the length of punctum at 100 μm below the surface were significantly lower in the punctoplasty group compared to the DCR group(p=0.034, 0.028, respectively)
Conclusion
For the first time, the measurements of the size of the punctum of healthy participants is presented. The size of the punctum of patients with epiphora complaints’ is smaller compared to patients without epiphora complaints’. AC- OCT can be a helpful tool to diagnose who needs lacrimal drainage system surgery and who can wait. This tool may be even more useful in area were an adnexal consultant is not available and telemedicine can be used.
