Isolated Rosai Dorfman Disease (RDD) of Lacrimal Sac- A Rare Finding

Purpose
To describe the 1. Clinical and histopathological findings of lacrimal sac RDD. 2.Role of fine needle aspiration cytology and its advantage in RDD
Methods
A 52-year-old lady presented with swelling of the lower eyelid of 2-months duration which was associated with pain and discharge. On clinical examination, a firm immobile mass was noted in the left lacrimal sac area with left submandibular (SMN) lymphnode swelling. On CT-scan imaging, a soft tissue mass in the lacrimal sac area with erosion of the underlying bone was noted. Incision biopsy of sac mass and fine needle aspiration cytology (FNAC) of SMN was performed.
Results
Histopathology revealed fibro-collagenous tissue infiltrated with chronic inflammatory cells and showed few lymphophagocytosis. FNAC of SMN revealed reactive lymphoid cells admixed with engulfed lymphocytes and plasma cells by the histiocytes (emperipolesis), S-100, CD68 and CD1a immune stain was done on incised sac tissue in which S-100 clearly demonstrated the emperipolesis and confirmed the diagnosis of RDD of lacrimal sac .
Conclusion
RDD is one of the non-Langerhans cells benign histiocytosis and rarely reported in the lacrimal sac. The prognosis is good with standard protocol and management. FNAC is cheap and help in rapid diagnosis of disease of lymphnode swellings.
