Endoscopic DCR using the Fusion® ENT navigation system

Purpose

In endoscopic endonasal dacryocystorhinostomy (DCR), surgery is often performed by determining and confirming the position of the lacrimal sac based on pre-operative computed tomography (CT) or nasolacrimal duct insertion of an intraoperative fiber optic light pipe. We assessed the feasibility of using the Fusion® ENT navigation system (electromagnetic guidance system) with endoscopic DCR in non-identified cases of the lacrimal sac.


Methods

Three patients (canalicular obstruction, orbital fracture reduction for multiple facial trauma due to a traffic accident, and acute dacryocystitis) underwent endoscopic DCR using the Fusion® ENT navigation system (Medtronic Co.) In preparation for use of the Landmar X/Fusion protocol, all patients underwent preoperative non-contrast CT. The lacrimal sac was confirmed after registration using the magnetic Fusion® ENT navigation system.


Results

In each case, the position of the lacrimal sac was confirmed while viewing three-dimensional CT images in real time during endoscopic DCR. The maxilla and lacrimal bones were precisely removed, and the bony window was created without using the fiber optic light pipe.


Conclusion

The Fusion® ENT navigation system proved especially useful in cases wherein it was difficult to confirm the lacrimal sac during endoscopic DCR.
