Lacrimal fistula in a girl with Goldenhar syndrome- how to treat?

Purpose
We present a 2 y/o girl with Goldenhar syndrome, who has a left sided lacrimal fistula connecting to the nasolacrimal duct. There is constant mucopurulent discharge form the opening and it affects cosmesis. CT scan with contrast (dacryocystography) was performed which showed the fistula connection with nasolacrimal duct. The aim was to review the literature on frequency of this lesion and the best surgical option.
Methods
Literature research using Pudmed.
Results
Lacrimal fistulas are rare supernumerary lacrimal canaliculi that connect the skin to the lacrimal system. Goldenhar syndrome or also known as oculo-auriculo-vertebral spectrum is a rare congenital condition characterized by abnormal development of the eye, ear and spine. We did not find any previous reports of lacrimal fistulas in Goldenhar syndrome.
Conclusion
Lacrimal fistulas can cause constant mucopurulent discharge from the opening and skin irritation. We propose that fistulas originating from the nasolacrimal duct should the excised. Simple closure of the fistula is insufficient.
