Necrotizing Canaliculitis: Describing a New Entity with its Clinical and Microbiological Aspects

Purpose

Describing a new entity of Canaliculitis that causes acute tissue necrosis, as well as reporting the clinical and microbiological spectrum in this specific group of patients.


Methods

A retrospective chart review was used to report patients with the presentation of Canaliculitis with acute tissue necrosis at King Khaled Eye Specialist Hospital between (2020-2023). The data collected includes demographics, type of organism, antibiotic sensitivity, response to antibiotics therapy and management.


Results

A total of five patients, three males and two females. All patients were adults, and they had no known predisposing factor for developing Canaliculitis. The presentation was fulminant, from the start of the symptoms onset to the necrosis period ranges from 1-3 days. One patient had consecutive bilateral involvement. The isolated organisms were : Pseudomonas aeruginosa in one patient, staphylococcus aureus in two patients and Methicillin-Resistant Staphylococcus aureus in two patients. All treated with necrotic tissue debridement, empirical antibiotics coverage and irrigation with povidone-iodine, 10%.


Conclusion

Necrotizing Canaliculitis is a newly described entity that was not described in the literature before apart from the first case that we reported earlier. Being familiar with such rare but possible serious presentations can yield better diagnosis and faster management response.
